Trinity’s Care Connection Ministry is committed to assisting our members and their families facing illness or other hardship connect to services within our church and community. This committee will provide leadership to:
* Coordinate the work of this ministry with the overall ministry of the congregation
* Conduct assessment visits with members, their families or caregivers
* Research and gather information about church and community resources
* Keep a list of church resources, member volunteers, and community services
* Train member volunteers to be available for short and long term support that may include respite for caregivers, phone calls, visits and cards of encouragement, grief support, coordinating community resources, transportation to church or appointments, meal preparation and delivery and grocery shopping
* Schedule the trained volunteers to support members in their time of need

If you or a family member would like to learn how the Care Connection Ministry might be able to assist you, please contact the church office (715-356-2255) or fill out the form provided to the right. Our pastors will forward the request to one of the Care Coordinators who will then contact you to set up an assessment visit to help determine the church or community resources that might best meet your needs. The Care Connection Ministry will respect your personal and family’s need for privacy and confidentiality and will not seek compensation for these services. While this ministry cannot guarantee it will be able to find a resource to meet every need, the ministry will be committed to being an additional source of friendship, support, compassion and prayer through the time of illness or other hardship. |

Volunteer to Serve
Do you enjoy preparing meals? Are you able to provide rides to appointments or church? Do you enjoy visiting with others and giving encouragement? Do you have extra time to run errands and deliver groceries? Trinity's Care Connection Ministry needs you! Volunteer a little, a lot or whatever your time allows.
